Seven Oaks Hospice Care is committed to providing patients and their families with the resources they need to make the hospice experience as understandable and comfortable as possible. In addition to the services directly offered by Seven Oaks, patients and families may find the below community resources to be useful.*

Hospice Organizations

The National Hospice and Palliative Care Organization (NHPCO) is the largest nonprofit membership organization representing hospice and palliative care programs and professionals in the United States. The organization is committed to improving end of life care and expanding access to hospice care with the goal of profoundly enhancing quality of life for people dying in America and their loved ones.

The Pennsylvania Hospice Network (PHN) was established in 1979 and incorporated in 1980 to provide an organizational structure for hospice programs in the Commonwealth of Pennsylvania. The first meeting, held in 1980, was sponsored by the American Cancer Society. Today, PHN has grown to more than 200 organizational and individual members state-wide.

Caring Connections

Caring Connections, a program of the NHPCO, is a national consumer and community engagement initiative that provides free resources and information to help people make decisions about end-of-life care and services before a crisis.
