Our team of trained and caring professionals provides patients and families with the following services:
Supervision by Hospice Physician
Medical care (especially the creation and implementation of an individualized Plan of Care) is closely supervised by a licensed physician.
Skilled Nursing Care
Nursing care is provided by caring and highly skilled registered nurses. Nurses supervise the individualized Plan of Care, provide direct care, and provide detailed and helpful instructions to the patient and his or her family or other caregiver.
Medications for Pain and Symptom Control
Each patient receives an individualized regime of medication to control pain and symptoms.
Nutritional and dietary needs are a vital part of the Interdisciplinary Team in developing the best plan of care for the patient
Medical Equipment and Supplies
Seven Oaks provides patients with the equipment and supplies which enable them to be comfortable, safe, and functioning to their fullest capacity.
We provide patients with certified nursing assistants (CNAs) who assist patients with daily living and personal care.
Medical Social Services
We provide each patient with a social worker who helps the patient and his or her family understand and navigate financial, emotional, psychosocial and other issues.
Physical, Occupational and Speech Therapy
When indicated by the Plan of Care, Seven Oaks provides patients with physical, occupational and speech therapists.
We provide a counselor who offers spiritual counseling tailored to the unique needs of each patient.
Seven Oaks provides a bereavement counselor who comforts families after the passing of a loved one.
Seven Oaks has a team of volunteers who provide patients and families with companionship and other assistance.
